The Conversation Project is dedicated to helping people talk about their wishes for end-of-life care. The program was started by a group of health care organizations who, along with the Institute for Healthcare Improvement, understanding discussing death and dying is difficult but needed to be able to respect people’s wishes for end-of-life care.

Realizing it is difficult to think about end-of-life process, yet to talk with someone you love and care for, The Conversation Project is dedicated to helping people talk about their wishes for end-of-life care. The Conversation Starter Kit is a useful tool to help people have conversations with their family members or other loved ones about their wishes regarding end-of-life care.

The tool is specifically designed to help families and loved ones of people to start talking with your loved ones openly and honestly, before a medical crisis happens, providing everyone a shared understanding about what matters most to you at the end of life. Sharing your wishes for end-of-life care can bring you closer to the people you love.

The Conversation Starter Kit (http://theconversationproject.org/starter-kits/ )gives you a step-by-step guide for what to say and what to do. It provides you with an outline and issues to think about when planning to have “The Conversation.” It also offers “ice breakers” or phases to help you start the conversation as well as suggest next step after.
